This optimizes and fixes bugs in our logic for maintaining a set of
running context servers, based on the combination of the user's
`context_servers` settings and their installed extensions.

This PR exposes context server settings to extensions.
Extensions can use `ContextServerSettings::for_project` to get the
context server settings for the current project.
The `experimental.context_servers` setting has been removed and replaced
with the `context_servers` setting (which is now an object instead of an
array).

This PR adds support for context servers provided by extensions.
To provide a context server from an extension, you need to list the
context servers in your `extension.toml`:
```toml
[context_servers.my-context-server]
```
And then implement the `context_server_command` method to return the
command that will be used to start the context server:

This PR reworks how the Assistant Panel references slash commands,
context servers, and tools.
Previously we were always reading them from the global registries, but
now we store individual collections on each Assistant Panel instance so
that there can be different ones registered for each project.

This PR makes the `Buffer::apply_ops` method infallible for
`text::Buffer` and `language::Buffer`.
We discovered that `text::Buffer::apply_ops` was only fallible due to
`apply_undo`, which didn't actually need to be fallible.

This will help us as we hit issues with the /workflow and step

import our refinements back into the source tree when we're ready.

This pull request introduces collaboration for the assistant panel by
turning `Context` into a CRDT. `ContextStore` is responsible for sending
and applying operations, as well as synchronizing missed changes while
the connection was lost.
Contexts are shared on a per-project basis, and only the host can share
them for now. Shared contexts can be accessed via the `History` tab in
the assistant panel.

This PR replaces the `lazy_static!` usages in the `paths` crate with
`OnceLock` from the standard library.
This allows us to drop the `lazy_static` dependency from this crate.
The paths are now exposed as accessor functions that reference a private
static value.

This PR extracts the definition of the various Zed paths out of `util`
and into a new `paths` crate.
`util` is for generic utils, while these paths are Zed-specific. For
instance, `gpui` depends on `util`, and it shouldn't have knowledge of
these paths, since they are only used by Zed.
